Get started with Troy

Troy works the moment you open it. A few minutes of setup makes your first notes noticeably better: teach Troy your voice, tell it your specialty, and pick the dictating style that fits your day.

1. Before you start

DictationWorks on any iPhone that runs Troy. No download needed.
AI notes and Ask TroyNeed an iPhone with 8 GB of memory (iPhone 15 Pro or newer) and a one-time 2.5 GB download.
Image analysis, longer notesNeed 12 GB of memory (iPhone 17 Pro and newer).
SoftwareThe latest iOS gives you the best speech recognition and AirPods support.

Have about 3 GB of free storage and a Wi-Fi connection for the AI download, and a quiet room for the minute it takes to record Voice ID.

2. Install and open Troy

1

Install from TestFlight

Troy is in beta. Use the TestFlight invitation from CodeBlueMD to install it. Not invited yet? Join the beta.

2

Read and accept the beta terms

Scroll to the end, then tap I Acknowledge & Accept. Troy is beta software and is not FDA-cleared, so always review a note before it goes into a chart.

3

Connect a computer, or skip for now

If you have a SafeKey, plug it in and tap Connect SafeKey. Troy asks for Bluetooth and pairs on its own.

No SafeKey yet? Tap Continue without a computer. You can record and review on your phone and connect later in Settings → SafeKey. If your EMR runs in Chrome, the free browser extension does the same job with no hardware.

3. Add on-device AI notes

4

Download Troy AI (optional, 2.5 GB)

On the Ready to dictate screen you can tap Start dictating right away, or Add AI notes · 2.5 GB download to turn your dictation into HPI, A&P and SOAP notes.

To add it later: Settings → Download Troy AI. It's a one-time download, and after that the AI runs fully offline on your iPhone.

Plug in, stay on Wi-Fi, and keep Troy open until it says the model has loaded. To switch AI notes on or off later, use Structure my notes in Settings → Advanced.

4. Set up Voice ID

Voice ID learns what you sound like, so Troy can tell your voice from a patient's, a nurse's, or the TV in the room. It's the single best thing you can do for accuracy in a busy clinic.

5

Record four short passages

Go to Settings → Teach Troy → Voice ID — Teach Troy Your Voice, or tap Set up Voice ID on the main screen.

  • Read the 3 enrollment passages and 1 check passage aloud, about 10 seconds each. They're fictional clinical text.
  • Do it somewhere quiet, using the microphone you'll dictate with (iPhone or AirPods), held at your usual distance.
  • Speak at your normal pace and volume. Don't over-pronounce.
  • Use the same microphone for all four passages.

When it's done you'll see Voice Enrolled. Your voiceprint stays on your iPhone.

If Troy suggests re-recording, or the main screen says Check Voice ID for this microphone after you switch mics, tap the Voice ID row again and re-record. It takes about a minute.

6

Choose how strictly Troy filters other voices

In Settings → Advanced → Voice isolation:

SettingBest for
OffA quiet office where only you speak. Transcribes everything the mic hears.
Light (default)Most clinics. Reduces background noise and checks it's your voice in Dictation.
StrongBusy rooms with nearby conversation. Filters more, but very short or overlapping voices can still get through.

For the strictest option, turn on Strict doctor-only delivery. Troy then types only phrases it's sure came from you and holds anything uncertain for you to review.

If you see "Voice uncertain — automatic typing paused", nothing is lost: review the held text when you finish recording.

5. Tell Troy about your practice

7

Specialty, billing and signature

Open Settings → My Practice:

  • Medical Specialty: improves drug-name recognition and tailors AI feedback to your field. Choose from Family Medicine, Internal Medicine, Cardiology, Neurology, Nephrology, Pulmonology, Gastroenterology, Endocrinology, Emergency Medicine or Psychiatry.
  • Billing Country: US or Canada. It's set from your phone's region; change it if needed. In Canada, Ontario (OHIP) is supported now.
  • Signature: e.g. "Jane Doe, MD". Say "SafeKey sign" to type it into the EMR.

6. Choose how you dictate

8

Set it up the way you work

OptionWhereChoices
Mic buttonSettings → DictatingTap to talk (default) or Hold to talk, which works like a PowerMic: hold while speaking, release to pause.
Pause lengthSettings → DictatingQuickest, Quick (default), Normal or Relaxed. Pick Relaxed if Troy cuts in while you're thinking.
Where words goMain screenType to computer types into your EMR as you speak (needs SafeKey or the extension). Review on phone lets you check everything first.
Note formatMain screenDictation (word-for-word), HPI, A&P, SOAP, or Encounter (ambient visit note, with patient consent).
Send without reviewSettings → DictatingOff by default. Leave it off so you review each note before it's sent.

7. Your first dictation

9

Allow the mic, then talk

Tap the microphone. The first time, iOS asks to use the Microphone and Speech Recognition; allow both. Speech recognition runs on your iPhone.

  • In "Type to computer" mode, say your punctuation ("period", "comma", "new paragraph"). Review and the AI note formats add punctuation for you.
  • Medical phrases stay as words: "colon cancer" and "last menstrual period" won't turn into punctuation.
  • Using AirPods? Pick them from the mic menu on the main screen. The (HD) option gives the best quality.

8. Voice commands worth knowing

10

Edit and navigate without touching the screen

SayWhat happens
"scratch that"Erases what you said since your last pause.
"delete that"Erases the last word.
"new line" / "new paragraph"Starts a new line or paragraph.
"period", "comma", "question mark", "colon", "semicolon"Inserts the punctuation.
"SafeKey tab"Jumps to the next field in your EMR.
"SafeKey sign"Types your signature.
"dot RP", "dot normal", "dot tele"Expands a shortcut: return precautions, normal exam, telehealth consent. Edit or add your own in Settings → SafeKey → Shortcuts.

SafeKey commands work in Dictation with Type to computer on.

9. Teach Troy your words

11

Fix a word once, and Troy remembers

When you correct a word in a note, Troy asks Remember this fix?. Tap Remember and it writes it your way from then on.

For names, drugs or terms Troy keeps missing, use Settings → Teach Troy → Teach a word: say the word three times and tick how Troy heard it. Your list lives under My word fixes.

12

Ask Troy

Tap Ask Troy next to the note format to ask a clinical question, or open a saved note from History and ask about it. It uses the on-device AI, so nothing leaves your phone. Answers are for reference only, so use your own clinical judgement.

10. Tips and getting help

Missing characters in the EMR?Set Settings → SafeKey → Typing speed to Safe / Citrix.
Lots of background voices?Re-record Voice ID with the mic you use and try Voice isolation: Strong.
Cutting you off mid-thought?Set the pause length to Normal or Relaxed.
Encounter modeGet consent, put the phone between you and the patient, and pause 1–2 seconds when the speaker changes.
💬
Something not right? Go to Settings → Help → Report a problem. It sends us a debug log and your device details so we can fix it fast. By default, the log doesn't include what you dictated.

Speech recognition and AI notes run on your iPhone. Always review notes before they reach a chart.
